What Are T4, T3, and Reverse T3?
The thyroid gland is a butterfly-shaped gland located at the front of the neck. It produces two main thyroid hormones: T4 and T3.
T3 is the active thyroid hormone. It works inside the cells to increase metabolic rate, pulse rate, body temperature, and energy production.
T4 is considered a storage hormone. On its own, it is largely inactive, but it can be converted into the active thyroid hormone T3. This conversion takes place mainly in the liver.
Under normal circumstances, a small amount of T4 is also converted into reverse T3, also called rT3. Reverse T3 is an inactive thyroid hormone.
What Is Reverse T3?
Reverse T3 is an inactive form of T3 that is produced from T4 instead of active T3.
Normally, the body produces only a small amount of reverse T3. But when reverse T3 production rises too much, it can create problems. Reverse T3 binds to thyroid hormone receptors on the cells, but unlike T3, it does not increase metabolic activity.
In simple terms, reverse T3 can lower the metabolic rate by blocking the effects of active T3 at the cellular level.
This is why high reverse T3 can create an anti-thyroid effect.
Why Is High Reverse T3 a Problem?
High reverse T3 can be problematic because it interferes with the action of active thyroid hormone.
Instead of allowing T3 to stimulate energy production, reverse T3 competes for access to the same receptors. The result may be a lower metabolic rate, even when thyroid blood tests appear normal.
This can contribute to symptoms such as:
fatigue
low body temperature
slow pulse
brain fog
constipation
weight gain
dry skin
hair loss
low mood
poor stress tolerance
In other words, a person may appear to have enough thyroid hormone on paper, while still experiencing symptoms of low thyroid function because reverse T3 is blocking the action of T3 inside the cells.
When Does Reverse T3 Increase?
Reverse T3 production tends to rise during times of physical or metabolic stress. This includes:
illness
inflammation
chronic stress
aging
starvation
low blood sugar
under-eating
excessive exercise
From an evolutionary perspective, reverse T3 is a protective adaptation. During famine, starvation, or severe stress, the body raises reverse T3 to slow the metabolic rate and conserve energy.
While this may be useful in a survival situation, it becomes a problem when reverse T3 remains elevated for too long and contributes to chronic low-thyroid symptoms.
Can High T4 Increase Reverse T3?
Yes. High levels of T4 can increase reverse T3 production.
This is especially important for people using T4-only thyroid medication.
Most people with low thyroid function are treated with synthetic T4-only medications. Common brand names include Synthroid, Levoxyl, Levothroid, Levo-T, L-Thyroxine, and levothyroxine.
The effects of synthetic T4 were originally tested in healthy young men. In these individuals, T4 often works well because they usually have strong liver function and can convert T4 efficiently into active T3.
However, this is not always the case in women, in older people, or in people who are sick, stressed, inflamed, undernourished, or have weaker liver function. In these situations, conversion of T4 into T3 may be impaired.
When T4 is not converted properly into T3, it can accumulate. That excess T4 may then be converted into reverse T3 instead, creating a stronger anti-thyroid effect.
Can T4-Only Medication Cause High Reverse T3?
For some people, yes.
A person taking T4-only medication may continue to experience symptoms of hypothyroidism even as the dose is increased. If the T4 is not being converted properly into T3, raising the T4 dose may worsen the problem by increasing reverse T3 production.
This means that some people on T4-only medication may feel worse, not better, especially if they already have poor thyroid conversion.
This is one reason some people continue to struggle with fatigue, low temperature, brain fog, and slow metabolism even while taking thyroid medication.

How Do You Treat High Reverse T3?

Why Reverse T3 Matters for Thyroid Health

A person may have:
normal TSH
normal T4
normal T3
and still feel hypothyroid if reverse T3 is elevated and blocking active thyroid hormone at the cellular level.
This is why a more complete thyroid assessment should not rely only on standard thyroid blood tests. It should also consider symptoms, pulse, temperature, stress levels, and the possibility of elevated reverse T3.
