Bioidentical hormones are often promoted as safe and a better choice than synthetic Hormone Replacement Therapy (HRT). However, according to Dr. Ray Peat, only some bioidentical hormones are truly safe. In this article, I explain how to identify which bioidentical hormones are helpful — and which ones to avoid.
What Is Hormone Replacement Therapy?
Hormones are signaling compounds made by the body's endocrine glands. They are released into the bloodstream and transported to different tissues, where they perform specialized actions. Hormones control important functions in your body, including:
- Metabolism and thyroid function
- Reproduction and fertility
- Sexual function
- Growth and tissue repair
When your hormones are out of balance, many harmful processes occur, including chronic inflammation and degenerative processes. A hormonal imbalance can cause sexual dysfunction, infertility, weight gain, fatigue, mood swings, premature aging, and much more. Supplementing with selected hormones can help people whose bodies do not make enough of a certain hormone, or whose hormone levels are unbalanced.
What Is Synthetic Hormone Replacement Therapy?
The hormonal compounds found in standard Hormone Replacement Therapy (HRT) are not bioidentical. This means they are similar — but not identical — to the natural hormones they replace. In order for a pharmaceutical company to patent a chemical compound, they need to alter its molecular structure slightly so that it differs from its natural form. This is because it is not possible to patent a natural substance.
Many different estrogenic substances are used in birth control pills. They have estrogen-like actions but are not chemically identical to the body's naturally produced estrogen. Similarly, pharmaceutical companies produce a range of different progesterone-like molecules called progestins. These compounds are similar but not identical to the natural progesterone produced by the body.
What Are Bioidentical Hormones?
Bioidentical hormones are hormone supplements that are chemically identical to the ones made by the body. Bioidentical hormone therapy with pregnenolone, progesterone, estrogen, and testosterone is available. Some bioidentical hormones are produced by pharmaceutical companies, others are sold by compounding pharmacies, and some are produced by health food companies. Because natural substances cannot be patented, there is relatively little commercial incentive to promote them.
Are Bioidentical Hormones Better Than HRT?
Many people ask whether bioidentical hormones are better than synthetic HRT. The answer depends on which bioidentical hormone you are considering. Some bioidentical hormones are safe and very helpful, while others are extremely harmful. Bioidentical hormones are often promoted as safe across the board — but this is only true for bioidentical pregnenolone and progesterone. Supplementing bioidentical estrogen or testosterone, on the other hand, carries serious risks.
Is Bioidentical Estrogen Dangerous?
According to Dr. Ray Peat, estrogen is a harmful hormone when present in excess. Only a small amount of estrogen is produced in the female ovaries. Both men and women are capable of producing large amounts of estrogen in various tissues when the body experiences stress or inflammation. Excess estrogen promotes premature aging, infertility in both men and women, chronic inflammation, and low thyroid function — reducing this burden is a core part of the lifestyle work I teach in my book Roadmap to Metabolic Health.
An estrogen deficiency is very unlikely. Even in female menopause, estrogen is not truly deficient — rather, it is the decline in progesterone production that drives the symptoms of menopause. You can learn much more about the relationship between progesterone and estrogen in my Hormone Rebalance Intensive™. The dietary principles that support hormonal balance are covered in my book How to Boost Metabolism with Food and my Metabolic Nutrition Mastery course.
Because estrogen is fundamentally a stress hormone that produces a wide range of harmful effects, supplementing any estrogenic substance — whether synthetic HRT or bioidentical estrogen — is extremely risky.
Is Bioidentical Testosterone Dangerous?
Many people believe that supplementing with testosterone improves sex drive and muscle strength. However, the opposite can be true if you supplement too much testosterone or if your body is under significant stress. In these situations, testosterone is easily converted to estrogen — which, as explained above, carries a range of negative effects. For this reason, I do not recommend using testosterone in any form, including bioidentical testosterone.
Dr. Ray Peat's Safe Bioidentical Hormones
According to Dr. Ray Peat, supplementing bioidentical progesterone and pregnenolone is safe and beneficial in cases of stress, aging, and chronic inflammation. Both progesterone and pregnenolone are synthesized in large quantities during youth and decline with age — which is why they are often referred to as youth-associated hormones.
Progesterone and pregnenolone are the body's natural protective substances. They both have anti-aging and anti-stress properties. Supplementing natural bioidentical progesterone and/or pregnenolone can help address stress, aging, chronic inflammation, estrogen excess, thyroid problems, infertility, and much more.
You can learn much more about the interplay between different hormones — and how to use progesterone and pregnenolone safely — in my Hormone Rebalance Intensive™. Best Brands of Bioidentical Progesterone and Pregnenolone
It is important to know that many different brands of bioidentical progesterone are available — but not all are equally effective or safe. I only recommend Dr. Ray Peat's progesterone oil. You can learn more about why this is the best natural progesterone supplement on the market in my blog post: Dr. Ray Peat's Progesterone.
In my Hormone Rebalance Intensive™, I provide detailed guidance on how to purchase and correctly use Dr. Ray Peat's progesterone oil, as well as a high-quality pregnenolone supplement — including dosing, timing, and what to expect.
Final Thoughts
Not all bioidentical hormones are created equal. While pregnenolone and progesterone offer genuine protective and restorative benefits, bioidentical estrogen and testosterone carry serious risks and I do not recommend them. Understanding the difference — rooted in Dr. Ray Peat's metabolic research — is one of the most important steps you can take for your long-term hormonal health.

Frequently Asked Questions
What are bioidentical hormones?
Bioidentical hormones are hormone supplements that are chemically identical to the hormones produced naturally by the human body. They include bioidentical versions of estrogen, progesterone, pregnenolone, and testosterone — and are distinct from synthetic progestins and estrogens used in conventional HRT.
Are bioidentical hormones safer than synthetic HRT?
According to Dr. Ray Peat, the answer depends entirely on which hormone. Bioidentical progesterone and pregnenolone are safe and beneficial. However, bioidentical estrogen carries the same risks as synthetic estrogen — because estrogen itself is a stress hormone regardless of its molecular source.
Why is bioidentical progesterone recommended by Ray Peat?
Dr. Ray Peat considered natural progesterone one of the most important protective hormones. It opposes estrogen, supports thyroid function, reduces inflammation, promotes sleep, and has anti-aging properties. Unlike synthetic progestins, natural bioidentical progesterone does not carry cardiovascular or cancer risks. I cover how to use it effectively in my Hormone Rebalance Intensive™.
What is pregnenolone and why is it beneficial?
Pregnenolone is the master precursor hormone — it is the starting material from which progesterone, DHEA, and other hormones are made. It declines with age and stress. Supplementing pregnenolone can support hormonal balance, memory, stress resilience, and anti-aging pathways according to Dr. Ray Peat's research. I explain how to use pregnenolone safely in my Hormone Rebalance Intensive™.
Is bioidentical testosterone safe to use?
Dr. Ray Peat advised against testosterone supplementation in most cases. Excess testosterone — especially under stress — is readily converted to estrogen via the enzyme aromatase. This can worsen hormonal imbalance rather than improve it. I do not recommend testosterone supplementation for this reason.
